2018 Parental Consent for Annual Census

The Federal Act to Promote the Education of the Blind was enacted by Congress in 1879. This act is a means for providing adapted educational materials to eligible students who meet or function at the definition of blindness. In order to meet this goal, the Indiana Educational Resource Center (IERC), on behalf of the Indiana Department of Education, conducts an annual registration of eligible students through the Indiana Center for Accessible Materials (ICAM). 

To be included in the Federal Quota program, eligible students must be registered in an annual census each January. The annual census is for those students who meet or function at the definition ONLY. This requires the exchange of personally identifiable student information (PII). The information is only collected to meet the reporting obligations to the U.S. Department of Education, Office of Special Education Programs, and other entities as required by law.
The U.S. Department of Education, per the Family Educational Rights and Privacy Act (FERPA), requires parental consent to release a student’s personally identifiable information. Districts must have on file a signed consent from parents or guardians allowing staff from your school district to provide names, birth dates, school districts, level of visual functioning, primary language of learner, and the primary and secondary reading media to the American Printing House for the Blind.
